General

How do I enroll in coverage?

As a new hire

You’ll have 30 days from your hire date to enroll in your benefits.

After your I-9 paperwork is completed and processed, you’ll get a Workday notification (no later than 72 hours of your I-9 being approved) with instructions on how to enroll.

If you want medical, dental or vision coverage, or if you want to contribute to a Health Savings Account (HSA) or Flexible Spending Account (FSA), or if you want to purchase disability benefits or any additional life insurance for yourself or your family, be sure to complete your enrollment during your 30-day window.

Unless you make an active election to decline coverage, you’re automatically opted into short-term disability coverage (you'll pay for this coverage out of your paycheck) and to the 401(k) Retirement Plan at a 2% contribution amount from your paycheck.

To cancel your Short-Term Disability enrollment, contact the Benefits Enrollment Center at 1-844-347-8941 and request to make a New Hire Enrollment or Correction, or do it online.

To update your 401(k) enrollment or contribution, contact Empower at 1-844-411-4015 or online.

During Annual Open Enrollment

Each year, you’ll have the opportunity to review and update your benefit elections for the upcoming year. You can change your medical coverage, add dependents or update contributions to accounts like your HSA or FSA.

Open Enrollment usually takes place in the fourth quarter of each year for benefits that are effective January 1 of the following year. You’ll be notified when the open enrollment period begins and ends. If you want to make changes before or after the open enrollment period, you must have a qualifying life event to make election changes.

Some benefits don’t require sign up during Open Enrollment. Your 401(k) through Empower includes automatic enrollment at a 2% contribution rate as a new hire, and will increase every year by 1% unless you opt out of that feature. You can start, stop or change your deferral elections at any time. Benefits, like the Employee Assistance Program (EAP) through SupportLinc, are available year-round without needing to opt in.

After a qualifying life event

Certain big moments in life, like getting married or divorced, having a baby, losing coverage elsewhere, or turning 26, allow you to make benefit changes outside of Open Enrollment. In most cases, you’ll need to make updates within 30 days of the event happening.

Remember you’ll need to submit documentation verifying certain events and/or dependents on a timely basis. If you don’t, your elections won’t take effect or will be reversed.

How do I update or change my coverage?

  • Health Savings Account (HSA) contributions or a Commuter Account can be updated any time during the year. To update your HSA or Commuter Account, contact the Benefits Enrollment Center at 1-844-347-8941. HSA contribution changes take place on the first of the month following your changes.

Annual Open Enrollment is a good time to review your current elections, add or remove dependents, and update contributions to accounts like your HSA or Flexible Spending Accounts (FSA). It’s also a great time to make sure the beneficiaries you’ve elected for your Life Insurance, 401(k) and HSA are current.

How can I see what I’m enrolled in?

You can view many of your benefits by logging into the Benefits Enrollment Center or calling 1-844-347-8941, Monday through Friday, 7 a.m. to 7 p.m. CT.

To see your 401(k) elections, visit Empower or call 1-844-411-4015, Monday through Friday, 8 a.m. to 9 p.m. CT.

Health

How do I get an ID card?

Medical & prescription drug

You can download your medical and prescription drug plan ID cards on your provider’s website or by downloading the mobile app.

Most offices will let you send them an email with a copy of your card if you use a digital version, rather than needing a paper copy.

Dental

You don’t need an ID card to see your dentist, but you do have access to your digital ID cards from my.cigna.com or the Cigna mobile app. You can share, print, email or save your ID cards to your mobile wallet.

Vision

If you’re visiting a VSP provider, no ID card is necessary! But if you’d like a copy of your ID card, you can print one from your VSP member account.

How do I find an in-network provider?

Medical & prescription drug

You can find an in-network provider or pharmacy by downloading your plan's mobile app or by visiting your plan’s website:

Dental

You can find in-network dental providers at my.cigna.com.

Vision

You can find participating VSP providers at VSP.com.

If you aren’t enrolled in a medical plan yet and want to see if your providers are in network:

  • For the UMR PPO or HDHP medical plans: Call Accolade at 1-866-417-0197 and stay on the line until you get Ava, the virtual assistant. You’ll be asked what you’re calling about, and you’ll need to provide the last four digits of your Social Security number, date of birth and ZIP code. Then, you’ll be transferred to a live person. They’ll ask for your name and date of birth and confirm you aren’t in the system. Let them know you’re a candidate or eligible for Caliber Holdings and they’ll assist you with questions about network coverage and any other question about the plans.
  • For Surest Plan: Visit the Surest plan site.

How do I find a virtual care provider?

You can find virtual care providers by by downloading your plan's mobile app or visiting your plan’s website:

Hinge Health offers virtual physical therapy if you’re covered under the UMR or Surest Plans – all without leaving your home.

If you’re experiencing neck, shoulder, back, hip, knee issues or looking for pelvic floor support, Hinge may be able to help you without having to go to a physical therapist. And the best part of Hinge is that you can complete your visits virtually – right from your own home.

Who do I go to with a question about a healthcare bill?

You can view your FSA balance by logging in to HSA Bank (you'll need to create an account if you haven't already).

How do I change my FSA contribution?

FSA contributions can only be adjusted mid-year if you experience certain qualifying life events. The Benefits Enrollment Center can help you if you're eligible to update your contribution due to a qualified life event. Contact them at 1-844-347-8941.

How do I find out how much coverage I have for a treatment or procedure I need to have done?

Coverage varies depending on the type of service and plan you have. The plan documents and the medical overview are great places to start for a general understanding of what you may need to pay.

What’s the best way to get in touch with my doctor?

Many providers have online portals that allow you to directly message a physician or care provider if you have questions. You can also call and leave a message for your provider, if you don’t have a login to your portal or if your physician doesn’t have a web service.

What should I do if I’m not sure where I should go for care?

If you’re unsure of where to go, virtual care is a great, low-cost place to start. Your virtual care provider can help assess your situation and will be able to help you understand the next steps for care, if any.

What if I have a medical emergency?

All of our medical plans cover three levels of care — virtual care, urgent care and emergency room visits — so you have options no matter what's happening.

The emergency room is for serious or life-threatening situations that need immediate attention, like chest pain, difficulty breathing, severe injuries, signs of stroke or uncontrolled bleeding. If you think you’re experiencing a medical emergency, call 911 or go to the nearest emergency room right away. Don’t wait.

Virtual care is the most convenient option for minor, non-emergency issues you can address from home, like a cold, flu, rash, sinus infection or a quick prescription refill. You can connect with a provider by phone or video, often within minutes and at a lower cost than an in-person visit. If necessary, these providers will call in a prescription for you night or day.

Virtual care is great when you have a sick child in the middle of the night, when you’re traveling or if you wake up not feeling well.

Urgent care is the right choice when you need to be seen in person but your situation isn’t life-threatening — think minor injuries, sprains, ear infections or a fever that isn’t improving. It’s faster and more affordable than the ER for these kinds of issues.

Be careful with urgent care centers – some will become “emergency rooms” after a certain time of night (and will bill as an emergency room) and others bill as emergency rooms 24/7/365. Always ask about how much you will pay and how the visit will be billed BEFORE you receive treatment.

Traveling? If you’re more than 100 miles from home, you have 24/7 access to emergency assistance through New York Life. They can help arrange transport to another hospital or back home. Call 1-347-708-1824. Remember that if you’re traveling and outside of your plan’s network area, going to an out-of-network emergency facility should be considered in-network when billed.

How can I get care if I’m traveling for work or personal reasons?

Wherever you are, you’ll still have access to care.

If you’re dealing with something minor, virtual care is usually the easiest option while traveling, and urgent care is a good choice if you’d rather be seen in person. Access virtual care or find a local, in-network urgent care through your plan’s mobile app or website:

If you have a serious or life-threatening emergency, head to the nearest emergency room or call 911 right away. Emergency care is covered anywhere, though out-of-network costs can vary depending on your plan and where you are when you get treatment.

If you’re diagnosed with a medical condition, your medical plan includes resources to help you along the way.

Depending on your plan, you and your family may have access to care coordinators, condition management programs, telemedicine, mental health support and other helpful tools.

How do I talk to my doctor?

You don’t need to have everything figured out before going to the doctor.

It’s okay to keep things simple and start with what brought you in, how you’ve been feeling, concerns about family history or any questions you have about your health. If it’s been a while since your last visit, your doctor can help you get back on track and talk through any recommended screenings, medications or next steps.

Some people also find it helpful to write down questions, symptoms or concerns ahead of time, so they don’t forget anything during their appointment.

The most important thing is starting the conversation and being honest about how you’re feeling.
