01

Continue certain benefits

You may enroll in COBRA if you need to continue your medical, dental or vision coverage.

02

End dates vary

Some benefits end as of your end date with Caliber and some run through the remainder of the month during which you leave.

03

Possible enrollment window

Some changes give you the opportunity to enroll in benefits. You’ll have 30 days to take action from your benefits eligibility date.

Decide

If your job change means you’re transitioning out of Caliber, you’ll receive details about your benefits end dates, COBRA options for medical, dental, and vision, retirement plan access and how to keep or transfer certain accounts.

Use

Use any remaining Flexible Spending Account (FSA) funds before your last day of work (submit any final claims incurred before your separation date within 90 days of leaving).

If you have a Health Savings Account (HSA), you can continue to use your HSA funds tax-free for eligible expenses even after you change jobs, though administrative charges may apply.

And your 401(k)? That’s yours to keep! You’ll be able to take that with you and can transfer your vested balance into another qualified investment account, even with a new employer. To access your 401(k), your status needs to be updated in Workday. Status changes are sent to Empower weekly, on Monday, for all changes that occurred the prior week.

Update

Confirm that your mailing address in Workday is correct to ensure you receive tax forms and any other communications we may need to send you. Remember to keep your address updated with Caliber until you receive your final W2 and 1095-C for the last year you worked.

Know

Be aware of when your benefits end:

  • Medical/prescription, dental, vision, and mental health benefits: These benefits end on the last day of the month in which your employment ends. You may elect to continue coverage under COBRA.
  • Flexible Spending Account (FSAs): Benefits end on your separation date. Only those expenses incurred on or prior to your last day of employment will be eligible for reimbursement. If you want to submit any final claims for reimbursement through your FSA, you’ll need to do so within 90 days following your separation date. If you want to keep your Health Care FSA and continue to access funds for services after your separation date, you must make an election to continue your FSA through COBRA. Note: Contributions to your FSA through COBRA are on a post-tax basis.
  • 401(k) Retirement Plan: Caliber will send your separation date to Empower, and you’ll receive a distribution packet from Empower at your home address.
  • Disability and Life Insurance: These benefits end on your separation date. You will receive information in the US Mail on continuing coverage at your own cost.
